Kitchen Supply Request Form

Please note:  A kitchen supply request may be made for internal ministry events that have at least 30 or more attendees. 

If you would like a copy of your submission, print page before you hit "submit".

If necessary, the Kitchen Manager will contact you with any questions and/or concerns.


VENDOR or CATERER (Certificate of Insurance must be supplied)

If this is a fundraiser, you must make arrangements with parish staff to drop the collected donations in one of the safes on property the DAY OF THE EVENT. No monies should ever leave the campus.

PLEASE NOTE: Please leave rooms arranged as you found it. Please do not use candles in carpeted meeting rooms or tape any decorations, signs etc. on walls without prior permission. There is no cooking allowed in the Community Room kitchen. Thank you for being good stewards!
