Baptism Registration Form

The information required on this form will be transferred to the permanent parish record of your child's baptism. Please note: A parent MUST complete the following information.

Child Information

The rite of Baptism consists of pouring holy water on the head while pronouncing the invocation of the Most Holy Trinity: the Father, the Son, and the Holy Spirit (immersion is available - but must be requested below and approved by the Pastor).

Baptisms are celebrated at 1:15 pm (after the 12:15 pm Mass).

Godparent Information

At least one godparent must be a confirmed, practicing Catholic over age of 16 and must submit a Sponsor Certificate.
