Application Process

Holy Name of Jesus Catholic School welcomes each family to a new school year filled with exciting learning experiences. The mission of Holy Name of Jesus School encourages a child-centered environment in which the needs of our students are met through a faith-based, excellent educational program.

The normal registration time for the next school year begins in February-March. Priority of acceptance into the school will be determined by the following criteria:

  • Currently enrolled families who are registered, supporting parishioners.
  • Currently enrolled non-parishioners.
  • New families who are registered, supporting parishioners.
  • New families who are non-parishioners.

If classroom space remains available after the initial in-house registration period, priority of acceptance will be determined on a first come, first served basis. If the maximum classroom or school enrollment is attained, a formal waiting list will be established at the school office, and classroom vacancies will be filled according to the above criteria.

For returning students, family accounts must be current, or the application will not be processed or accepted until such accounts are current.

New Students entering Kindergarten must be five (5) years old on or before September 1st of the new school year. This is the law in the State of Florida and must be followed by the school.  Children entering first grade must be six years old on or before September 1st and have successfully completed Kindergarten.

Prospective new students for grades 6-8 and their parents must be interviewed by the Principal or his/her representative. Placement testing may be required for any student.

New students, once accepted, are required to submit:

  1. Copy of latest interim or report card from the current school.
  2. HRS Form 680 Immunization Record.
  3. HRS Form 3040 Physical Examination.
  4. Certified copy of a birth certificate.
  5. All Applications forms (contact School Office) and nonrefundable fee.

NOTE: Students coming from other Catholic or private schools must have cleared their accounts at the former school before beginning school at Holy Name of Jesus.

Returning families can re-enroll in FACTS family portal.  FACTS selects one parent as the designated re-enroller.  You should have received an email.  If you want to change the re-enrollment parent, please contact the school.  If you have not yet made a FACTS account, please contact the school so they can email you the steps to make a FACTS account and re-enrolling your child.

Please note all families must register with FACTS to complete the registration process. FACTS is the largest provider of tuition payment plans in the industry, giving students and families from educational institutions at all levels a better way to manage education costs over time. With a FACTS Tuition Payment Plan, you can choose a convenient payment option to suit your needs. Instead of one lump tuition payment, smaller payments can be made over time.

Tuition Assistance

Endowment Scholarships and HNJ Parish Financial Aid:  Tuition assistance may be available to parishioners who demonstrate true, financial need. Tuition assistance applications may be obtained through Donna Lonski, Life Center Office, dlonksi@hnj.org, 321-773-2783, x114. The parish uses FACTS, a second party financial assessment company, to determine family eligibility for tuition assistance, and all information submitted for review is strictly confidential. Tuition assistance granted for one year does not guarantee that it will be granted every year.

Step-Up for Students:  Step-up for Students goal is to give families a choice in their child's education. These scholarships are awarded on a first come-first serve basis. For more information, please visit www.stepupforstudents.org or call 877-735-7837.

Immunizations

The Department of Health with compliance of Florida State Law (FS1003.22) requires all students have current/updated immunizations. They also require that the student have a recent physical exam on file. Failure to do so will result in your child no longer being able to attend school as per Florida Law. Immunization records are reviewed by the Health Department at the beginning of each school year.

Pre K

  • 4 DTP/DTaP
  • 3 Polio
  • 1 MMR
  • 3 Hepatitis 
  • 1 Varicella (or verified chicken pox)
  • 1 HIB

K-6th Grade

  • 5 DTP/DTaP 
  • 4 Polio 
  • 2 MMR

7-8th Grade

  • 5 DTP/DTaP

Immunizations may be obtained from a private physician or free from:  Brevard County.  Routine childhood immunizations are provided free of charge for children through 18 years of age, regardless of family income. Please bring your child's shot record with you.  Brevard County Health Department clinic locations in Titusville, Viera and Melbourne provide immunizations without appointments. Normal business hours are Monday - Friday 8 AM - 5 PM. Immunizations are provided between 8 AM - 4 PM.  For a nominal fee, Brevard County Health Department also offers travel, pneumonia, flu and other adult vaccines.  This flu season, flu shots will be available through the health department on a limited basis at low cost for adults. Flu shots are available on a walk-in basis, Monday through Friday from 8:00 a.m. to 4:30 p.m. at all three clinic sites.  Pneumonia shots are available year round.  For more information, call Brevard County Health Department (321) 634-6336.
